Field Trip Permission Slip
I request that my daughter/son be allowed to go to Union Square on Nov.
2, 2005. I give permission for my daughter to miss school so that she may
participate in the most important lesson of her life; learning that
history is in their hands, and that they can be part of determining the
future. I expect that my daughter will not be punished or stopped for
participating in this historic event.
I release, and hold harmless _______________ School and its employees from
any and all liability for any and all harm, as they are not sponsoring or
endorsing this event.
I understand the students will meet there or travel there by whatever means.
